A dedicated full-time Nurse Practitioner is sought to join an interdisciplinary care team, providing comprehensive, patient-centered primary care to frail and elderly populations. Collaboration with physicians to deliver primary care services is essential. Comprehensive participant assessments (initial, ongoing, and as-needed) will be performed. Individualized care plans will be developed, implemented, and managed. The practitioner will provide acute illness evaluation and treatment. Participation in discharge planning for hospital or long-term care transitions is required. Home visits will be conducted when necessary to ensure continuity of care. Assistance in the development and implementation of clinical policies and procedures will be expected. Active participation in interdisciplinary team meetings is necessary. Care must focus on geriatric and frail populations. Accurate and timely documentation of all clinical encounters must be ensured. Participation in provider on-call rotation will be part of the responsibilities. Support for a compassionate, patient-centered care environment is crucial. Clean malpractice and license history is highly preferred. This position is only open for permanent placements. Fresh graduates will not be considered. Visa support is not available for this role.
