Hospice Education Liaison — RN, Hospital Partnerships

joliethospice is looking for a Clinical Liaison based primarily at NW Palos to provide education on hospice and palliative care. This role involves assessing needs for hospital education and establishing relationships with physicians while ensuring support for patients and families during discharge. Candidates must have at least 2 years of relevant experience, a valid Illinois RN license, and be a licensed driver.
