Deliver impactful patient care in the community as a Home Health Physical Therapist Assistant (PTA). This contract opportunity allows you to directly support individuals in their homes, helping them regain independence and mobility under the direction and supervision of a licensed Physical Therapist. Youll thrive in this field-based role with an emphasis on autonomy, compassion, and skillful, patient-centered service.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Daily travel to patient homes within your assigned territory

What youll do:

• Deliver skilled physical therapy treatments based on individualized care plans developed by the supervising PT
• Guide patients in therapeutic exercises, gait and balance training, transfer techniques, and functional mobility activities
• Monitor, document, and report patient progress, responses to therapy, and any changes in condition to the care team
• Provide vital education to patients and caregivers, addressing safety, use of assistive devices, fall prevention, and home exercise routines
• Maintain timely and accurate records using Home Care Home Base (HCHB)
• Collaborate with a multidisciplinary team, participating in case conferences and contributing insights for exceptional patient outcomes

Required Qualifications:

• Active PTA license in good standing
• At least 1 year of recent Home Health experience as a PTA (field experience preferred)
• Familiarity with Home Care Home Base (HCHB) and OASIS documentation is desirable
• Reliable transportation, a valid drivers license, and current auto insurance
• Strong observational, communication, and time management skills
• Comfort working independently and efficiently organizing a field-based schedule

$1,542 - $1,631/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
