Responsibilities Perform skilled nursing visits: assessments, reassessments, and care coordination in patient homes Develop and evaluate Plans of Care (POC) and document changes clearly Complete accurateOASISdocumentation and charting inHCHB (Homecare Homebase) Provide wound care, IV care/venipuncture, catheter/drain care, and medication education Educate patients and caregivers on disease management, home safety, and self-care Coordinate with physicians and interdisciplinary teams; ensure continuity of care Ensure compliance with infection control (OSHA), HIPAA, and payer documentation requirements Travel across assigned territory (approximately a 60-mile radius) and participate in on-call coverage as assigned Qualifications Active, unrestrictedRegistered Nurse (RN)license (Michigan) CurrentCPR/BLScertification 2+ years home health experiencepreferred StrongHCHBexperience required and strongOASISexperience required Valid driver’s license, reliable transportation, and ability to travel daily Strong assessment skills, documentation accuracy, and time management CHPN certification preferred (not required)
