Health Plan Clinical Pharmacist I - MTMP & CPAS Focus

Kelsey-Seybold Clinic, part of the Optum family, is seeking a clinical pharmacist to support Pharmacy Quality Initiatives across our Houston-area locations. You will conduct medication reconciliation, patient counseling, and chart reviews, collaborating with physicians to ensure appropriate therapies and adherence. Responsibilities include MTMP, CPAS, adherence programs, and MTMP-related audits, with emphasis on accurate documentation in EPIC and strong customer service delivery. #J-18808-Ljbffr UnitedHealth Group
