About the Role

Lehoel is seeking a Registered Dietitian Nutritionist (RDN/CDN) to provide onsite nutrition services in Rockland County in partnership with Social Care Network (SCN). You will deliver individualized nutrition assessments, care plans, and education to the clients and families we serve, working collaboratively with our care team. This is a flexible part-time role (approximately 20 hours/week, with room to grow) ideal for a dietitian who wants meaningful, community-centered work.

Responsibilities

- Conduct nutrition assessments and develop individualized nutrition care plans

- Coordinate with the Social Care Network (SCN) and broader care team on client goals

- Provide nutrition education and coaching to clients and caregivers

- Monitor progress and adjust care plans as needed

- Maintain accurate, timely documentation and meet program/billing requirements

Qualifications

- Registered Dietitian Nutritionist (RDN) credential - required

- Certified Dietitian Nutritionist (CDN) state certification - required/preferred

- Bachelor's or Master's in Nutrition, Dietetics, or related field

- Experience coordinating with care-management or community-based programs a plus

- Strong communication and documentation skills

Compensation & Schedule

- $50-$65 per hour, depending on experience

- Part-time, approximately 20 hours per week (room to grow)

- Onsite in Rockland County, NY
