Required Qualifications:

• Active and unrestricted LPN license
• At least one year of recent home health experience, specifically with geriatric patients
• Proficiency in HomeCare HomeBase (HCHB) EMR
• Strong clinical assessment and documentation abilities
• Dependable transportation for regular home visits
• Ability to work independently while efficiently managing a caseload
