Job Details Job Locations: US-NY-Gloversville Category: Case/Care Management Position Type: Full-Time Work Arrangement: Onsite ID: 2026-6934 Overview Helio Health is looking for a Case Manager to support its mission of promoting recovery from the effects of substance use and mental health disorders and other health issues. This role is vital to the management and coordination of services to patients/residents participating in, or who have completed our treatment programs. This is a Full‑Time position located at our ESSHI SP SRO Fulton Program . In addition to Helio Health's comprehensive benefits package, full‑time hires for this position are eligible for a Sign‑On Incentive up to $1,500 . Pay: $23.00 – $25.00 per hour. Responsibilities Maintain a case load and perform primary case work responsibilities including: Screening, intake, orientation Referrals Counseling Crisis intervention Patient/resident education Case management Reporting and record keeping Consultation with other professionals Coordination of care with other community providers Discharge planning Train staff, volunteers, and students/interns Perform Quality Assurance activities Provide services according to agency policies, procedures, program guidelines, and governmental regulations. Monitor provision of services when assigned. Evaluate programs and identify strengths/weaknesses. Assist in problem resolution and implementation of supervisor‑approved program modifications. Participate in program administrative and clinical committees as assigned. Represent agency to the community. Other duties as assigned by supervisor. Qualifications Associate's degree in a related human services field required; Bachelor's preferred. Two years of experience working with behavioral health and/or substance abuse issues.
