Job Summary

RYZE at the Ridge is seeking a dedicated and compassionate Psychiatric Rehabilitation Services Coordinator (PRSC) to provide group and individual psychiatric rehabilitative and case management services to residents with a history of psychiatric needs who require long-term care and stabilization. The PRSC works closely with the interdisciplinary team to develop, implement, and evaluate effective therapeutic services and recovery-focused programming.

Duties & Responsibilities

• Provide case management and counseling services to residents as part of an assigned caseload.

• Facilitate supportive treatment interventions to maintain or enhance residents' capacity for independent functioning.

• Provide crisis intervention as needed, including assessment of suicidal and homicidal ideation and implementation of appropriate stabilization measures.

• Facilitate psychiatric hospitalizations as needed.

• Participate in resident evaluations, care planning, and treatment meetings.

• Communicate with psychiatrists and other healthcare providers regarding residents' ongoing treatment plans.

• Conduct skills groups, psychoeducational programming, and counseling sessions.

• Coordinate with outside providers to ensure continuity of care.

• Participate in the orientation and training of new employees and students.

• Maintain accurate documentation in accordance with facility policies and regulatory requirements.

• Collaborate with the interdisciplinary team to support residents' treatment goals and overall well-being.

• Perform other duties as assigned.

Qualifications

• Bachelor's Degree in Psychology, Social Work, Human Services, or a related field required.

• Minimum of one year of supervised experience in mental health or human services required.

• Experience providing psychiatric rehabilitation or behavioral health services in a Long-Term Care, Post-Acute Care, or SMRF setting preferred.

• Knowledge of behavioral health treatment and psychiatric rehabilitation principles.

• Experience with computerized documentation systems, preferably PointClickCare (PCC).

• Strong communication, interpersonal, and organizational skills.

• Ability to work effectively with residents, families, and interdisciplinary team members.

• Ability to read, write, speak, and understand English.

• Ability to lift, push, pull, and move up to 25 pounds.

• Must successfully complete all pre-employment requirements.
