RN Case Manager: Care Coordination & Discharge Planning

RN.com is seeking a skilled RN Case Manager in the Los Angeles area to oversee care coordination, discharge planning, and patient-centered care planning in a dynamic hospital setting. The position requires an active CA RN license, BLS, and experience with hospital care coordination, ideally within CommonSpirit/Dignity networks.
