Field-Based Community Health Advocate

AbsoluteCARE Medical Center & Pharmacy

The Community Health Worker at AbsoluteCARE Medical Center & Pharmacy will connect members with resources, address social needs, and build trust in home, community, hospital, or clinic settings. You will enroll new members and guide them through services, reporting to the Community Team Care Manager and Engagement Director. This role involves in-person assessments, care plan development, coordinating with care teams, documenting outreach in Jiva and eCW, and meeting KPIs for member engagement #J-18808-Ljbffr AbsoluteCARE Medical Center & Pharmacy
