Ambulatory Nurse Care Coordinator- Full Time Days

Salary range: $38.50 – $61.60 per hour, with placement determined by experience, equity, and union agreements. Description The Ambulatory Nurse Care Coordinator provides patient care support services and ensures high quality of care through education and robust care management coordination. The role acts as a liaison between patients, physicians, and clinical support staff, helping patients understand their medical conditions and health responsibilities. Responsibilities Serve as a patient advocate in an ambulatory setting and assist in navigating across various care settings. Address care coordination needs for high- and moderate-risk patients as well as assistance with low-risk or new patients. Review patient records, registries, reports, or other encounter or claims data to identify patients who may require care coordination. Assess, document, and address clinical, psychosocial, or financial barriers to effective patient care. Consult with multidisciplinary teams to develop, implement, and evaluate care plans specific to the patient’s needs. Provide effective triage care using in-depth clinical knowledge and skills in the area of expertise. Offer educational support to patients and family members regarding medical processes, procedures, treatments, medications, and health management. Ensure timely continuity of care by proactively outreaching to patients post-discharge, managing referrals, facilitating transitions of care, and coordinating community resources. Manage high-risk patient cases to minimize readmission rates and reduce care costs, ensuring consistent high-quality care across settings. Develop care plans that address overall health goals aligning with patient choices and values; inform patients of preventive care practices and support activation of care plans. Maintain patient health information, including medical records and other pertinent data, to inform providers and care team members. Monitor care quality, patient and physician satisfaction, and participate in continuous quality and performance improvement initiatives. Provide after-hours on‑call support for urgent care coordination issues. Perform other related duties as directed or required. Qualifications Required Registered Nurse license – State of Illinois Professional Nurse. Bachelor of Nursing Degree (BSN) with 3 to 5 years of relevant clinical experience, or BSN within two years of hire. 3 years of experience (5 years preferred) working with chronic conditions, evidence-based medicine, care coordination, and psychosocial/behavioral health factors affecting outcomes. Willingness to travel as needed. Preferred 3 years of experience in Care Management or Care Coordination. Case Management certification or willingness to complete it upon meeting eligibility criteria. Experience with ambulatory operations and ambulatory nursing care. Experience with process improvement.
