Hybrid In Home Nurse Practitioner/Physician Assistant

As an Advanced Practice Clinician (APC), you will be part of a transformative program designed to provide personalized care and support to our patients, including those with complex and chronic needs, all in the comfort of their own homes. The APC will ensure the delivery of high-quality clinical and home-based patient care while promoting optimal health outcomes and providing cost-effective solutions that adhere to Alignment Health’s policies, as well as state and federal regulations. You’ll assess, develop, and coordinate care options tailored to the needs of our members, who are typically 65 and older. Home and virtual visits range from 45 to 60 minutes and can include initial visits, annual check-ups, or additional support to address medical and social challenges.

Responsibilities

• Deliver high-quality clinical and home-based patient care in collaboration with the patient and care team.
• Assess patient needs, develop care plans, and coordinate care options tailored to older adults.
• Conduct home visits and virtual visits, typically 45 to 60 minutes, and provide focused, compassionate care during initial visits, annual check-ups, or when additional support is needed.

Qualifications

• Education: Master’s degree from an accredited NP Program or PA program
• Licensure: Active California state Nurse Practitioner or Physician Assistant license
• Licensure/Certification: Active RN license and furnishing number (must, upon hire) for NP
• Board Certification: Active Nurse Practitioner Board Certification or Physician Assistant Certification
• Experience: Preferred – one (1) year of prior clinical or home care experience; EMR experience preferred; experience in care of older adult (geriatric) patients preferred

Education and Licensure Details

• Also required: Valid California driver license and current automobile insurance

Other

Pay Range: $113,332.00 - $169,999.00. Pay range may be based on market location, education, responsibilities, experience, etc.
