Job Description: 
Essential Job Duties:
* Perform in-person comprehensive assessments including initial, semi-annual and unscheduled assessments and continuous medical coverage to participants
* Collaborate with physicians and IDT to develop care plans for each participant
* Collaborate with physicians and IDT in discharge planning from acute or long-term settings
* Help maintain care of participants in the nursing home, making regular visits as dictated by participant need
* At the direction of the Medical Director, review outcomes and utilization data, including hospitalizations, and use of specialists to establish best practices and policy improvements
* Take part in rotating night and weekend call

Job Requirements:
* Graduation from an Accredited School of Nursing or Physicians Asst studies
* Certificate as a Nurse Practitioner with Prescriptive Authority or PA
* NP State License or PA License
* Minimum of two years of experience working with the senior population.
* Current knowledge of chronic care/geriatric issues and best practices.

Skills and Certifications: 
*Certificated as a Nurse Practitioner OR Physicians Asst
*Licensed in California
*2 years of NP/PA exp.
*Geriatrics experience (preferred)
