Cross Country Nurses is seeking a travel nurse RN Home Health for a travel nursing job in South Dennis, Massachusetts. Job Description & Requirements Specialty: Home Health Discipline: RN Start Date: 09/28/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Job Summary Home Healthcare Nursing Located remotely, this position involves performing comprehensive nursing assessments upon patient admission, including physiological, physiopsychological, environmental factors, and health history. The role focuses on developing and revising individualized care plans, ensuring quality of care, and collaborating with interdisciplinary teams to meet patient needs. Job Details Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: Current RN license in Massachusetts, successful passage of VNA pre-hire medication test, CPR certification; home care and IV experience, computer skills, and BSN preferred; minimum one (1) year of nursing experience required or 6 months with a BSN. Schedule: Day shift, 8 hours per shift, minimum of 40 hours per week. Responsibilities include conducting patient assessments, developing care plans, documenting patient progress, coordinating discharges, and providing supervision of home health aides every 14 days. Completes admission and discharge paperwork promptly and accurately, ensuring all documentation reflects skilled care and patient education. Notifies relevant healthcare providers of significant patient condition changes and records all clinical interactions. Requires continuous evaluation of care effectiveness and adherence to case management objectives.
