Behavioral Health Community Partnership Manager - Humboldt County

Behavioral Health Community Partnership Manager (Behavioral Health Provider Quality Manager) Sign on Bonus: $2,500 License Requirement: Candidates must possess a California state licensure (RN, LCSW, LMFT, LMHC, LPC, or Licensed Psychologist). Location: Must reside in Humboldt County. Role includes virtual work and client visits in the field. Travel: Requires travel to Humboldt County field locations. Schedule: Pacific Standard Time. How You Will Make An Impact Establish relationships with BH providers to improve clinical and quality outcomes for members. Collaborate with internal clinical and quality departments to achieve high‑quality care and HEDIS performance. Build positive working relationships with providers, state agencies, advocacy groups, and other stakeholders. Conduct routine meetings with strategic providers face‑to‑face, telephonically, and via Web‑Ex to support provider training and issue resolution. Serve as liaison between providers and Carelon clinical, quality, provider strategy, operations, and claims departments for interdepartmental coordination. Support regional and corporate initiatives such as the Carelon Select Provider program and clinical innovation. Create and maintain linkages between providers and community services to improve transitions of care. Partner with network providers and stakeholders to operationalize innovative programs and resources. Analyze provider reports on cost, utilization, and outcomes; present data and highlight trends. Identify data outliers and opportunities for improvement for individual providers. Spot high‑performing providers interested in new programmatic incentives or payment models. Participate in identifying and launching innovative pilot programs, measuring efficacy and outcomes. Contribute to high‑quality program ideas in the local market to drive value. Provide consultation to providers for clinically complex members as needed. Surface clinical and quality issues to regional teams and help address concerns. Conduct quarterly physician record reviews with network providers. Assist with provider orientations and training events in the region. Attend all County BH provider meetings, in person or via telephone or Web‑Ex. Minimum Requirements MA/MS or higher in a behavioral health field and a minimum of 10 years of progressively responsible healthcare experience, including at least 5 years in a behavioral health setting or an equivalent combination of education and experience. Current, valid, independent, and unrestricted license (RN, LCSW, LMFT, LMHC, LPC, or Licensed Psychologist). Preferred Skills, Capabilities, and Experiences Behavioral health experience. Managed care experience. Willingness to travel to work sites and other locations as necessary. Salary Range: $75,920 – $113,880 (California).
