Hybrid Care Manager: Life Plans & Advocacy for I/DD

Care Design New York

Care Design New York is seeking a Care Manager to help people with intellectual and developmental disabilities identify and realize their goals by developing, implementing, and monitoring person-centered Life Plans. The hybrid role requires home visits, office trainings, and remote work, with driving may be needed outside NYC. Responsibilities include advocating for member decisions, leading meetings, updating Life Plans, coordinating supports, and ensuring timely documentation. #J-18808-Ljbffr Care Design New York
