\nWe are seeking experienced Registered Nurses (RNs) with strong assessment, care coordination, and member education skills to support a Disease Management program. Candidates with prior Case Management experience, telephonic care management experience, and Medicaid knowledge are highly preferred.
\n \nRequired Qualifications:
\n• Active Pennsylvania RN License
\n• Disease Management experience
\n• Telephonic Case Management experience
\n• Strong assessment, care coordination, and member education skills
\n \nPreferred Qualifications:
\n• CCM (Certified Case Manager)
\n• Medicaid experience
\n• Previous Case Management experience
\n \nAdditional Requirement:
\n• Candidates must complete the Care Manager Assessment in Glider.
