HEALING HAVEN HOME CARE SERVICES LLC
Benefits: Competitive salary Flexible schedule The Social Worker — provides intensive, housing focused case management to Veterans experiencing homelessness or at risk for homlessness. Each Social Worker will maintain a caseload of no more than four active Veterans, allowing for frequent contact, individualized support, persistent outreach, and hands‑on assistance through every stage of the housing process. The Social Worker conducts intake and comprehensive assessments; develops and regularly updates individualized service plans (ISPs); provides housing navigation; coordinates medical, behavioral health, benefits, legal, employment, transportation, and other supportive services; monitors progress; and facilitates placement into safe, appropriate, and sustainable permanent housing. The position serves as a consistent point of contact and advocate for each Veteran, using trauma-informed, person-centered, culturally responsive, harm-reduction, and housing-focused practices Flexible work from home options available. #J-18808-Ljbffr HEALING HAVEN HOME CARE SERVICES LLC
