As a Case Manager within the Specialty Care Clinic at the Georgia Department of Public Health, you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgia residents in a variety of ways.
Job Responsibilities
Master's degree in social work and counseling from an accredited college or university and eligibility to be licensed.

As a Case Manager within the Specialty Care Clinic at the Georgia Department of Public Health, you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgia residents in a variety of ways.
Job Responsibilities
- Ensure each client is eligible for Ryan White Services.
- Conduct psychosocial assessments.
- Provide referrals to appropriate community services.
- Advocate for clients and assist with removing barriers to care.
- Prepare and update service plans as outlined in the Ryan White Case Management Standard Operating Procedures.
- Collaborate with medical staff to develop effective treatment plans to enhance health outcomes.
- Maintain knowledge of current best practices and implement interventions based on empirical evidence.
- Adhere to the NASW Code of Ethics and clinic protocols.
- Provide timely and thorough documentation of interventions, phone calls, and services provided.
- Provide crisis intervention services and referral to appropriate care.
- Assist clients with applying for medication assistance programs.
