Job Summary:
A Home Health Registered Nurse (RN) position is available in the Massachusetts area. This role involves assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Responsibilities include performing admission processes and case management duties in accordance with organizational policies and standards of community health practice, utilizing electronic documentation and interdisciplinary communication.

Job Details:

• Schedule: Day shift, 8 hours per day, with a minimum of 40 hours per week.
• Required Qualifications: Candidates must have at least 1 year of Certified Home Care experience, 1 year of Case Management experience, and must be able to travel to cases as assigned. Experience solely in hospice, private care, or working as a Case Manager without experience in a certified home health agency does not meet the qualifications. All applicants must have a minimum of 1 year of relevant experience.

• Additional Information: The position covers multiple units within the region, including Cape Ann, Essex, Gloucester, Manchester, Rockport, Ipswich, North Ipswich, Amesbury, Bradford, Byfield, Georgetown, Groveland, Haverhill, Merrimack, Methuen, Newbury, Newburyport, Rowley, Salisbury, and West Newbury.
