Minimum Qualifications:

• Education : Bachelor's degree with progressive experience or a Master's degree in Physical Therapy.
• Licensure : Must be licensed to practice as a Physical Therapist in Illinois.
• Experience : Previous experience in home healthcare or familiarity with conditions of participation is preferred.
• Additional : Must have a current driver's license and reliable transportation to travel to and from patient sites.
