Hybrid RN: Home Health Case Manager & Field Assessments

TALENT Software Services is seeking a Registered Nurse for a field-based, hybrid role serving clients in the Phoenix area. The position requires 10% onsite travel and 90% remote work, with travel to client sites as needed. You will complete nursing referrals, coordinate with interdisciplinary teams, and document services per policy.
