Care Coordinator

Overview Care Coordinators play an important role in the quality of care, as they serve as the bridge between our providers, facility care teams, and patients/patient families. Responsibilities Work under the supervision of a Nurse Case Manager. Chart creation with data entry of patient demographics, medications, and diagnosis. Process and maintain records with compliance of HIPAA. Communicate daily with Providers, Nurse Case Managers, and other interdisciplinary team members involved in the patient’s care. Provide timely communication and follow‑up with patients, families, and healthcare providers regarding changes in patient condition. Conduct monthly wellness calls to patients. Provide kind and professional support to patients and families. Facilitate patient access to appropriate medical and specialty providers as well as other care coordination team support. Other duties as assigned. Skills Problem-solving and critical thinking. Passion for patient care. Effective communication skills through all modes of communication. Strong work ethic that includes dependability and initiative. Detail‑oriented with organizational skills. Ability to be adaptable and manage time effectively. Ability to work both independently and within a team. Basic typing skills, computer, and previous EMR experience. Experience with Google Workspace applications.
