Registered Nurse- Clinical Care GuideLocation: Tucson, AZ 85718 (100% Onsite) Job Type: Direct Hire | Full-Time Salary: $85,000–$90,000 per year (W2)Job Summary: We are seeking a compassionate and experienced Registered Nurse (RN) clinical to join our care team as a Clinical Care Guide. In this role, you will coordinate specialist referrals, diagnostic testing, chronic disease management, and patient education while ensuring an exceptional patient experience. You'll work closely with providers and patients to deliver high-quality, coordinated care.Required Qualifications:Active Arizona RN licenseAssociate's Degree in Nursing (BSN preferred)Inpatient Case Management experience (Outpatient Case Management will also be considered)Experience with chronic disease management and patient care coordinationExperience coordinating specialist referrals and diagnostic testingElectronic Medical Records (EMR) experienceExcellent communication, customer service, and organizational skillsActive Arizona Fingerprint Clearance Card (or ability to obtain)Preferred Qualifications:BSNCase Management Certification (CCM)Experience with healthcare software platformsIf you're passionate about improving patient outcomes and delivering exceptional care, we'd love to hear from you!
