Hospice Case Manager, RN

Manages a nursing caseload as assigned by supervisor Makes an assessment of the patient and family's needs to develop the nursing care plan Initiates the admission and plan of care Confers with physicians and other hospice team members to develop the interdisciplinary Hospice plan of care Evaluates ongoing the patient and family’s psychosocial, spiritual, and physical needs Directs the care given by Home Health Aides, respite aides and volunteers Rotates through weekend, holiday, and 24-hour call as assigned by supervisor Requirements Graduate of an accredited school of nursing 2 years experience as a RN Preferred - Hospice or Home Health experience Required within 30 days of hire: National Provider BLS - American Heart Association Required upon hire: California Registered Nurse License Core Competencies Demonstrates expertise in nursing care management, including patient assessment, care planning, and interdisciplinary collaboration. Holds a California Registered Nurse License and possesses BLS certification, with a focus on hospice or home health care.
