Home Health Registered Nurse (RN) Case Manager

Full Time

Job Description:

• Responsible for care and documentation of care which meets reimbursement guidelines.
• Coordinate physical care of the patient with family, patient, physician and Hospice team.
• Communicate with patients/families at all levels.
• Communicate with all agency staff.
• Willingness to travel within the southside to see/meet with patients and their family.
• Minimal call volume.

Job duties:

Follows standard Nursing practices and implements agency policy and procedures.

Assesses patient's physical and emotional status, safety at admission and on an ongoing basis.

Develops comprehensive care plan based on each patient's assessment.

Provides direct care and gives instruction to nursing assistants.

Records and reports changes in condition to the physician.

Participates in IDT.

Documents orders in accordance with regulatory and agency requirements.

Job Requirements:

• Current RN licensure in state of practice, successful completion of a nursing education from an approved school of nursing with one year of clinical nursing experience and at least one year of case management experience. Hospice nursing experience is ideal.
• Knowledge of the framework, function, and organization of a home health and hospice care program and the nurse's role as a member of the patient care team.
• Working knowledge of the principles and practice of nursing according to nursing practice act, agencies policies and procedures, and reimbursement guidelines.
