Clinical Pharmacist

Job Description The clinical pharmacist will be a unit-based pharmacist that will verify the orders and provide clinical services for the area(s) for which they are assigned. They will be a drug information resource for the nurses, physicians, patients, and families. Responsibilities All general responsibilities of a Staff Pharmacist. Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals. Routine attendance on medical rounds (for decentralized shifts) with consistent documentation of pharmacist interventions. Deliver medication therapy management and ensure pharmaceutical care programs are appropriately integrated throughout the institution. Assist in development and implementation of facility-specific protocols, guidelines, and reference documents related to medication therapy management. Conduct Medication Use Evaluations (MUEs) and assist in making necessary changes to pharmacotherapy practice as needed based on MUE findings. Qualifications Required: Education: Pharm. D. or BS in Pharmacy Experience: Pharmacy practice residency or equivalent hospital experience (3 postgraduate years) Licensure/Certification: Pharmacy license BLS within 30 days of hire (ACLS and/or PALS if code attendance required); Board Certification in Pharmacotherapy (BCPS) About Us Embark on a rewarding career with Metrowest Medical Center - Consolidated hospital.
