Apply Today!Job Details: Schedule: 8 am - 5 pm Location: Lewisburg, WV Job Type: Contract Contract Length: Pay Range: $52/hr - $56/hr Pay offered is based on experience, expertise, credentialing, and education. Travel: Fayette County, Nicholas County Mileage Reimbursement: YesResponsibilities:Conduct comprehensive in-home physical therapy evaluations, assessing mobility, strength, balance, pain, endurance, and home safetyDevelop, implement, and regularly update individualized Plans of Care (POC) based on physician orders, patient goals, and measurable outcomesDeliver skilled physical therapy interventions to improve mobility, gait, balance, strength, endurance, and pain managementEducate patients and caregivers on diagnosis, safety precautions, fall prevention, home exercise programs, and use of medical equipmentPerform home safety assessments and recommend modifications to enhance safety and support independent functioningMaintain accurate, timely, and compliant documentation within the electronic medical record (EMR)Collaborate with interdisciplinary team members to coordinate care and optimize patient outcomesCommunicate changes in patient condition or barriers to care promptly to physicians and relevant stakeholdersEnsure compliance with payer requirements, including documentation standards and eligibility criteriaManage caseload efficiently while meeting scheduling, productivity, and visit timeliness expectationsMust have qualifications/experience:HCHB AND OASIS proficientWest Virginia PT licenseCurrent CPR certification.Valid driver's license, auto insurance, and reliable transportation.Experience in home health or related clinical settings preferred.Pride-Health offers eligible employee's comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance and employee discounts with preferred vendors.Equal Employment Opportunity Pride-Health is an equal opportunity employer.
