Job Summary

This 100% Licensed Clinical Social Worker position is for a full-time role in the UC Davis EDAPT clinic within the Behavioral Health Center at the UC Davis Medical Center. The goal of the UC Davis EDAPT clinic is to intervene as early as possible to support recovery for individuals with serious mental illness. The LCSW serves as an integral member of the UC Davis EDAPT team, providing screening, assessments, care coordination, information and referral to community resources, and other social work services designed to meet the complex needs of patients and families in the clinic setting. The responsibilities of this position are to provide case management/care coordination services in this coordinated specialty care (CSC) program by directly supporting individuals through navigating community-based services, resource linkage, and support in crisis management under the supervision of the program supervisor. They will support conversations on family needs identification (FNI), set case management goals with individuals, and facilitate access to community supports and resources for individuals and support persons. Additionally, they will assist with transitions from inpatient or higher levels of care to CSC, as well as support with discharge planning once their time in the program has concluded.
The LCSW shall adhere to and promote the ethics and values of the social work profession, using the NASW Code of Ethics as a guide to ethical decision making. The LCSW will advocate for the patient's right to self-determination, confidentiality, access to supportive services and resources, and appropriate inclusion in decision making that affects their health and well-being. The LCSW will be an active participant and work collaboratively with the multidisciplinary team by recognizing and offering services that address the biopsychosocial needs of the patient. The LCSW position requires a high level of clinical judgement, problem-solving skills, creativity, organizational and priority setting abilities, with excellent communication skills. The LCSW demonstrates the ability to fulfill these responsibilities through clinical assessment, accurate and sound judgement, and maintaining professional communication. The LCSW may be asked to participate in special projects and collect data to improve the quality of services delivered to our patients. The LCSW must be flexible as changes in technology, healthcare coverage, regulatory requirements, and our patient population may mandate changes to workflow, work standards, and work practices on an as needed basis.Apply By Date: August 8, 2026 at 11:59pm. - California Licensed Clinical Social Worker (LCSW)
- Valid California Board of Behavioral Sciences Licensed Clinical Social Worker (LCSW) license, active and unrestricted.
- Graduation with a master's degree from an accredited school of social work (MSW).
- At least 2 years of experience in a social service or healthcare setting (i.e., case management or social work).
- Knowledge of community resources and awareness of how to assist patients and families with linkage to resources is required.
- Knowledge and understanding of individual, child, and family development, human behavior and communication, the influence of culture and values in social work practice, psychotherapy, case management, and the relationship between social work practice and medicine.
- Knowledge and skills in the areas of individual, group, and family therapy.
- Solid biopsychosocial assessment abilities.
- Ability to perform in-depth chart analysis to obtain and communicate accurate information in a concise, informative manner
- Excellent clinical judgment and problem-solving skills.
- Knowledge of biopsychosocial and behavioral health treatment plans.
- Knowledge of chronic and acute disease, and how it influences patient and family functioning.
- Ability to handle demanding workload involving multiple tasks effectively and efficiently.
- Excellent written and verbal communication skills.
- Demonstrated ability to effectively collaborate with physicians and other healthcare and psychiatric providers, consumers and families, clinical and research staff, and other health care professionals is required.
- Experience interacting with people of diverse backgrounds, the ability to identify when differences in culture or language may impact communication, and knowledge of how to resolve those differences appropriately and with cultural humility is required.
- Knowledge of child, elder or dependent adult abuse and domestic violence reporting requirements, and other significant legislation and laws regulating social work practice, e.g., Tarasoff, patient confidentiality, etc., and the ability to make appropriate interventions based on law and policy, is required.
- Knowledge and experience with Windows based computers and associated software programs is highly recommended.
- Ability to work independently and organize a variety of concurrent activities, set realistic goals and objectives, plan ahead and meet deadlines without compromising quality
Preferred Qualifications
- Experience with assisting in psychiatric case management services including psychiatric hospitalization and linkage to community resources.
- Experience with electronic medical records.
- Previous experience working with family members of consumers with major psychiatric disorders.
- Experience with assisting in psychiatric case management services including psychiatric hospitalization and linkage to community resources is preferred.
- Spanish language skills are strongly preferred. Use of Spanish language skills in a professional context.
