Bilingual Care Coordinator - Hybrid Health Home & Advocacy
NYPCC
NYPCC is seeking a bilingual Care Coordinator to join our team in New York, working a hybrid schedule with on-site days at our Jackson Heights location. You will manage an 85+ caseload, develop comprehensive care plans, and coordinate various services to support clients’ wellbeing. The role requires a Bachelor's degree, experience with RMA and HARP clients preferred, and strong verbal and written communication to provide linguistically appropriate services.
