Pulmonary Program Coordinator
Job Summary:
New skilled nursing facility is seeking a dedicated and compassionate Pulmonary Program Coordinator to join our team in Castle Rock, CO. As a key member of our rehabilitation team, you will play a vital role in ensuring the successful implementation and coordination of our pulmonary program. About the Role:
As a Pulmonary Program Coordinator , you will be responsible for coordinating the daily operations of our pulmonary program, collaborating with healthcare professionals to develop and implement individualized treatment plans, and providing education and support to patients and their families. The ideal candidate will possess strong communication and organizational skills, with the ability to work effectively in a fast-paced environment.
Responsibilities:
Coordinate the daily operations of our pulmonary program, including scheduling and managing therapy sessions, patient assessments, and treatment plans
Collaborate with medical staff to develop and implement individualized treatment plans for patients with pulmonary diseases and conditions
Provide education and support to patients and their families on pulmonary rehabilitation techniques, disease management, and healthy lifestyle habits
Conduct patient assessments, including pulmonary function tests, and interpret results to inform treatment plans
Document patient progress, identify trends, and make data-driven decisions to optimize treatment outcomes
Develop and maintain relationships with healthcare providers and community organizations to ensure seamless transitions and coordination of care
Requirements:
1-2 years of experience in a pulmonary rehabilitation or related setting
Strong understanding of pulmonary diseases and conditions, as well as evidence-based treatment modalities
Excellent communication, organizational, and interpersonal skills
Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities
Current CPR certification and ability to lift up to 25 pounds
