PT - Physical Therapist

Outpatient Physical Therapist

This is a 13-week contract position.

5x8.0 schedule, 40 hours per week with a flexible schedule.

The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans in accordance with the patients' needs, goals, and physician direction. Responsible for the delegation and direction of all discipline-specific therapy services for those assigned patients. Ensures the design of functional treatment programs allows for effective return to community activities.

Responsibilities

• Completes a comprehensive evaluation of assigned patients.
• Utilizes clinical reasoning to develop appropriate treatment plans and goals focused on advancing patients toward discharge within an established length of stay.
• Maintains clear, concise, and timely documentation including goal writing and assessments.
• Participates in the student program by providing supervision and evaluating their progress.
• Actively creates a professional development plan with clinical management.
• Adheres to discipline-specific State Practice Act and maintains licensure in good standing.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1+ year of outpatient clinical experience required.
• Current CPR certification.
• No new graduates will be considered.
• Active and applicable state licensure for the location of work.
