HIV Care RN Case Manager - Holistic Health Navigator

Crusader Community Health

Crusader Community Health in Rockford, Illinois is seeking a Living with HIV Registered Nurse Case Manager. This role is responsible for coordinating medical interventions through specialty clinics and residential site visits, promoting optimal health, and applying a holistic approach to client services.

The ideal candidate will have an Associates’ Degree, a valid Illinois RN license, and CPR certification. Benefits include medical and dental coverage, paid time off, and a 403(b) retirement plan.
