Key Responsibilities

• Conduct thorough consultations with patients and collaborate with other healthcare professionals as necessary to develop an individualized plan of care.

• Perform detailed evaluations, including a review of patients' medical histories and assessments, ensuring accurate documentation of results and treatment recommendations.

• Plan and implement effective discharge strategies, ensuring that patients understand their follow-up care while maintaining precise documentation of patient progress throughout their therapy journey.

• Stay compliant with all regulatory benchmarks by completing necessary corporate and clinical training and adhering to state, local, and federal guidelines.

• Provide direct care to patients requiring physical therapy, ensuring clear documentation of the medical necessity for therapy.

• Engage in open communication with co-workers, patients, and family members to enhance cooperation and understanding in the healing process.
