Case ManagerWe are seeking a dedicated and experienced Case Manager to float across various inpatient units. This role will require flexibility and collaboration with healthcare teams to ensure optimal patient outcomes.ScheduleShift: Monday - Friday 08:30 - 17:00No call responsibilitiesWeekend Requirements: One weekend per month, with the flexibility to work a weekend day in place of a weekday shift if scheduledKey ResponsibilitiesProvide comprehensive case management services across multiple inpatient units.Collaborate with multidisciplinary teams to develop and implement patient care plans.Conduct assessments and evaluations to determine patient needs and appropriate services.Monitor patient progress and adjust care plans as necessary to optimize patient outcomes.QualificationsAcute hospital experience is required.Experience with Epic electronic health record system is mandatory.Minimum of 2 years of relevant case management experience preferred.The unit is open to accepting first-time travelers.Local candidates are welcome.Client Details State MI
