Job Description
Registered Nurse - Hospice

Schedule

7 on/7 off, with on-call hours from Monday to Friday, 5 PM to 8 AM, and 24 hours from 8 AM to 8 AM on Saturday and Sunday one week, then off the next week.

Assignment Details

Guaranteed Hours: 40 hours per week.
Contract Weeks: 14

Requirements

• Ample hospice experience
• Experience with on-call and admissions
• Experience using the HCHB electronic medical record system

Responsibilities

• Assess and identify patients' and families' needs for physical, psychosocial, environmental, safety, and developmental care.
• Document nursing assessments, planning, interventions, and patient outcomes.
• Develop and implement an individualized pain management plan.
• Participate in interdisciplinary team meetings to establish and revise care plans.
• Provide education and support to patients, families, and caregivers regarding treatment and care processes.
• Coordinate holistic, family-centered care across treatment settings to improve quality of life.
