Diversion Case Manager - Outpatient Behavioral Health
Community House Mental Health Agency
Community House Mental Health Agency in Seattle seeks a dedicated Diversion Case Manager to coordinate outpatient behavioral health services within the assigned diversion program. You will lead treatment planning, document progress notes, and collaborate with the care team to meet client needs and program contracts. You will work Monday–Friday, 9:00am–4:00pm, with trauma-informed practices, crisis intervention when needed, and a focus on high-quality, confidentiality-respecting care in King #J-18808-Ljbffr Community House Mental Health Agency
