Home Health Registered Nurse

The role involves full-time registered nursing in home health admissions and start-of-care visits, based primarily in Overland Park, Kansas, with occasional coverage in Missouri. The RN will conduct patient assessments, complete OASIS documentation, and collaborate with interdisciplinary teams to deliver personalized care plans. This position offers a stable, supportive clinical environment for experienced home health nurses.

Key Responsibilities:

• Perform home health admissions and start-of-care visits for new patients
• Conduct comprehensive patient assessments in the home setting
• Complete and document OASIS assessments accurately and in compliance with regulatory standards
• Utilize Matrix EMR for patient documentation and care planning
• Collaborate with interdisciplinary care teams to develop individualized care plans
• Educate patients and families on treatment plans, medications, and home health services
• Ensure all documentation meets state, federal, and agency compliance requirements
• Coordinate care with physicians, therapists, and other healthcare professionals
• Monitor patient conditions and communicate changes to appropriate clinical staff

Qualifications:

• Active Registered Nurse license
• Compact RN license or active licenses in Kansas and Missouri
• Graduate of an accredited nursing program (ADN or BSN)
• Minimum 1 year of nursing experience
• Minimum 1 year of home health or hospice experience
• Minimum 1 year of OASIS documentation experience
• Minimum 1 year of Matrix EMR experience
• Strong knowledge of home health regulations and documentation standards
• Strong clinical assessment and communication skills

Preferred Skills:

• Current Basic Life Support certification
• Experience performing admissions or start-of-care visits
