Registered Nurse (RN) - Case Management

Travel Registered Nurse (RN) Case Management

Schedule: Full-Time: 40 hours per week, 5 (8) hour shifts from 8:00 AM to 4:30 PM, as well as every third weekend.

Assignment Details: Contract Length: 13 weeks Guaranteed Hours: 40 hours per week

Requirements: Current licensure by the state Board of Nursing. Minimum of five (5) years of recent experience in clinical nursing or a related field (e.g., Utilization Review or Case Management). Previous experience in Care Management, Case Management, or Utilization Management required. Experience with performing 30+ reviews a day. Proficiency with screening criteria including Cerner, InterQual, and MCG. Excellent critical thinking and communication skills. Strong computer skills. ACM/CCM Certification is highly desired.

Roles & Responsibilities: Coordinate patient care and collaborate with healthcare teams to ensure optimal outcomes. Conduct assessments and reviews to determine patient needs and eligibility for services. Utilize clinical judgement to guide decisions regarding patient care management. Monitor treatment plans and adjust strategies based on patient progress and outcomes. Facilitate communication between patients, families, and healthcare providers. Maintain accurate documentation of patient interactions and care provided.
