Job Title: Transplant Coordinator Profession: Registered Nurse Specialty: Transplant Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum 2 years of experience required License: Valid nursing license required Certifications: BLS, Chemo (ONS/ONCC) Must-Have: Hematology/oncology experience is a must. Preferred experience in navigator/case management. Experience in acute hospital and outpatient clinic settings. Familiarity with pre-transplant evaluation and assessment. Understanding of post-transplant health maintenance and long-term care. Knowledge of transplant center protocols and waitlist criteria. Description: CROSS POSTING FOR JOB ID#: 3322071. RTO after booking is not allowed. Any call-offs or additional RTO on the unit could possibly result in a cancellation. Patients will have hematologic malignancies. The coordinator will follow cases from candidate identification through evaluation, cell harvest, and infusion or transplant. The coordinator may manage assignments of 8 or more patients concurrently. Organization and autonomy are essential for this role. Daily census could involve interactions with approximately 20 parties on behalf of the patient. Coordinator responsibilities include scheduling pre- and post-transplant testing and collaborating with the transplant team. Coordinators are required to monitor vital signs, weight, fluid balance, and graft function. Responsibilities also include reviewing and interpreting diagnostic and laboratory data. Candidates must demonstrate an understanding of absolute and relative contraindications to transplant. Coordination of medication adjustments post-transplant with providers is also necessary. Familiarity with organ-specific diseases is required. Job duties will include offering psychosocial support and education on lifestyle adjustments to patients and families. Continuous tracking of patient outcomes is a key responsibility. The unit is equipped with Pyxis and EPIC systems.
