Job Title: Transplant Coordinator Profession: Registered Nurse Specialty: Oncology Chemo Certified Duration: 13 Weeks Shift: Day Hours per Shift: 8 Hours Experience: Minimum 3 Years of Experience License: NC or Compact License Pending OK for Submittal Certifications: BLS, Chemo (ONS/ONCC) Must-Have: - Hematology/Oncology experience - Navigator/Case Manager experience Description: CROSS POSTING FOR JOB ID. RTO after booking is not allowed and is not allowed to be worked onsite. Any call-offs or additional RTO on the unit could possibly turn into a Cancel and DNR. Hematology/Oncology experience is a must. Navigator/Case Manager experience is a must. Minimum 3 years of experience is required. Traveler experience is accepted, including first-time travelers. Local candidates are accepted with a rate difference. Required work setting skills include acute hospital and outpatient clinic experience. Pre-transplant evaluation and assessment skills are required. Clinical assessment and skills include the interpretation of lab results and diagnostic tests related to transplant. Must gather and review medical history of potential or actual donors. Monitoring vital signs, weight, fluid balance, and graft function is essential. The role includes reviewing and interpreting relevant diagnostic and laboratory data. Professional knowledge and skills are necessary to coordinate all patient care needs before, during, and after transplantation. Understanding HIPAA and ensuring patient confidentiality is crucial. Patient and family education are important components of the role. Scheduling and tracking pre- and post-transplant testing is a requirement. Collaboration with transplant teams and adherence to transplant center protocols are necessary. Knowledge of post-transplant health maintenance and long-term care is important. Understanding universal precautions and waitlist criteria is essential. Experience with absolute contraindications to transplant and organ allocation processes is preferred. The role involves coordinating post-transplant care and monitoring patient progress. Providing psychosocial support and education on lifestyle adjustments to patients and families is essential. The position requires teaching medication adherence and post-operative care plans. Familiarity with organ-specific diseases is required. Identifying signs and symptoms of transplant complications is necessary. Performing comprehensive physical and psychosocial assessments is an important part of the role. Conducting medication administration duties, coordinating adjustments post-transplant with providers, and reviewing contraindications are needed. It is essential to track patient outcomes and report as per regulatory standards. Coordination of scheduling for the operating room and special procedures regarding cellular therapy is a part of the job. No weekend or call requirements are expected for this position. Candidates should be organized and able to work autonomously while managing multiple patient cases.
