Senior Case Manager II — High-Need Care Coordination

Family Health Centers of San Diego

Family Health Centers of San Diego is seeking a full-time social services professional to support high-need, medically underserved clients across multiple sites in San Diego. You will assess needs, develop formal care plans, coordinate services, and provide therapeutic interventions to improve health outcomes. Requirements include a Bachelor’s degree in a related field, 3 years' experience, and a valid California driver’s license; a Master’s degree is preferred. #J-18808-Ljbffr Family Health Centers of San Diego
