Outpatient Registered Nurse- Patient Aligned Care Team

Summary The Cincinnati VA Medical Center is recruiting for a Patient Aligned Care Team (PACT) Registered Nurse (RN) at its Georgetown - OH Community Based Outpatient Clinic (CBOC). The PACT RN executes responsibilities that demonstrate leadership, experience and creative approaches to management of complex primary care. Qualifications English Language Proficiency: Proficiency in basic written and spoken English is required. Graduate of an approved nursing school accredited by ACEN or CCNE. Or individuals in a masters‑level bridge program with equivalent coursework may become registered as a nurse with a state licensing board before completing the bridge program. Upon achieving a state license, appointment may be temporary and later converted to permanent upon completion of the bridge program. Foreign graduates with current, full, active and unrestricted registration meet the requirement; credit for higher education requires formal equivalency validation. Graduate Nurse Technician Exception: candidates meeting education but not licensure may be appointed as GNT on a temporary appointment while pursuing licensure. Grade determination scope‑education criteria apply (details available in online assessment). Preferred experience: two years of primary care experience. Physical requirements: standing, walking, stooping, bending, reaching, lifting; capable of moving clients up to 50 pounds and using mechanical devices; visual and auditory acuity necessary; ability to control or defend against emotionally ill clients. Responsibilities Provide gender‑specific direct veteran care and care coordination within the Primary Care Department. Manage assigned patient panels, utilize registries, and ensure appropriate access and evaluation. Provide health promotion, health education and coaching on wellness, disease prevention and chronic care management. Collaborate in development, implementation, evaluation and revision of policies, procedures and guidelines. Demonstrate performance and leadership that improves care for the Primary Care Department and facility. Provide and receive constructive feedback for improvement. Support colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Foster a safe and supportive environment conducive to professional development. Evaluate outpatient outcomes of evidence‑based decisions and practice changes for individuals, groups and populations. Paid Time Off: 50 days per year (26 days of annual leave, 13 days of sick leave, 11 paid federal holidays).
