Home Health RN / RN Case Manager

Location: Mesa County, Colorado
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Hours: 40 hours guaranteed
On-Call: Potentially 1 day per week and 1 weekend per month; not guaranteed
License Required: Active Colorado RN license
Mileage Reimbursement: $0.43/mile

Position Overview

We are seeking an experienced Registered Nurse with Home Health/Case Management experience to provide comprehensive care coordination and clinical support for high-risk patients throughout Mesa County.

The RN will assess patient needs, develop individualized care plans, coordinate services with healthcare providers, and help ensure patients receive the appropriate level of care and support.

Responsibilities

• Perform comprehensive assessments of high-risk patients.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare providers.
• Coordinate care transitions between hospitals, home health, physicians, and other levels of care.
• Work closely with physicians, social workers, discharge planners, and other healthcare professionals.
• Identify medical and non-medical barriers to care, including transportation and housing needs.
• Monitor patient progress and continuously reassess care plans.
• Provide health education, disease prevention, and health promotion support.
• Advocate for patients while protecting privacy and confidentiality.
• Maintain accurate documentation of clinical, functional, and care management outcomes.
• Identify potential risks and intervene quickly to help prevent adverse patient outcomes.

Required Skills & Experience

• Active, unrestricted Colorado Registered Nurse (RN) license
• Associate's or Bachelor's Degree in Nursing
• Previous Home Health and/or Case Management experience
• Strong knowledge of OASIS
• Wound care experience
• Experience with PICC lines
• IV therapy experience
• Knowledge of INR monitoring
• Ability to assess complex medical information and make appropriate clinical decisions
• Strong care coordination and patient communication skills
• Case Management certification preferred/required based on client guidelines
