Clinical Coordinator - Triangle Partial Program (LGBTQIA+ Program)

Partial Hospitalization Program Lead Clinician: Triangle Program HRI Hospital is seeking a full‑time Lead Clinician for the Triangle Partial Hospitalization Program (PHP). The program serves the LGBTQIA+ community and operates Monday‑Friday, 40 hours per week. HRI Hospital is a 66‑bed psychiatric facility offering adult inpatient services and several partial hospitalization programs. Responsibilities Actively identifies obstacles to achieving positive outcomes and works with providers, the treatment team, and managers to resolve them. Collaborates with patients, families, and outside agencies regularly to coordinate patient care and thoughtful discharge planning. Provides supervision and mentors other staff/clinicians as assigned. Actively engages in multidisciplinary team meetings to provide input regarding psychosocial problems and overall treatment objectives. Collaborates with the treatment team to provide effective clinical interventions and coordinates with the Assistant Case Manager to plan for discharge and after‑care needs. Assists with utilization review procedures for patients through accurate and thorough documentation of patient progress, ensuring payer requirements are met to help obtain authorization for covered days. Assists with administrative tasks to support the leadership team and participates in departmental leadership initiatives to support program growth. Experience: One year post‑master’s experience required; prior PHP or IOP, outpatient counseling, inpatient care coordination, and/or substance use treatment experience preferred. Licensure: LCSW, LICSW, LMHC, LMFT, or LADC required; LICSW or LMHC preferred. Passionate about working with the LGBTQIA+ community is strongly preferred.
