What You'll Do

• Evaluate each patient's physical abilities and functional limitations through comprehensive assessments and diagnostic tests.
• Develop personalized plans of care that target functional improvement, safety, and independencemodifying as needed in consultation with the physician.
• Deliver hands-on therapy in the home, using evidence-based techniques to help patients meet their mobility and strength goals.
• Educate patients and caregivers on therapeutic exercises, safe mobility, and the use of prosthetics, orthotics, and adaptive equipment.
• Document progress accurately and promptly in the patient's clinical record, maintaining clear communication with physicians and the interdisciplinary team.
• Ensure a safe environment for patients by practicing proper equipment handling, adhering to safety protocols, and collaborating with other care providers.

What You Bring

• Graduate of an accredited Physical Therapy program; DPT preferred.
• Current, valid Physical Therapy license issued by the New York State Department of Education.
• At least one year of post-licensing, practical experience as a licensed physical therapist.
• Clear communicator, with excellent critical thinking, and problem-solving skills.
• Passion for helping others regain confidence and independence through compassionate care.

Work Environment

• Home-based role providing care across assigned territories.
