Physical Therapist needed for a travel assignment in a skilled nursing facility in Seaford, Delaware. Start date 12/01/2026 for 13 weeks. Earn $2,454–$2,534 per week with guaranteed 35 hours per week. Shift is 5x8 Days (08:00–16:30). Work setting is a long-term care environment in a SNF.

Responsibilities
- Conduct comprehensive PT evaluations and ongoing reassessments for residents to determine need for therapy and track progress
- Develop, implement, and adjust individualized treatment plans focused on mobility, gait, balance, strength, and range of motion
- Deliver therapeutic interventions including therapeutic exercises, gait training, balance activities, and modalities within scope of practice
- Monitor patient responses, document progress, and communicate findings to the interdisciplinary care team
- Educate residents and families on home exercise programs, safety, and functional goals
- Ensure clinical documentation compliance, infection control, and safety standards in daily care delivery

Requirements
- Active Physical Therapist license in the United States or eligibility for licensure in the assignment state
- Ability to work in a skilled nursing facility environment
- Strong written and verbal communication skills; proficient charting and documentation
- Willingness to commit to a 13-week travel assignment and 8-hour shifts
- Preferred: experience in geriatric rehabilitation or SNF settings
