Clinical Care Manager — HIV Care & Navigation

Housing Works, Inc. is seeking a Clinical Care Manager to support the BEW (BeIntoHealth for Women) Project in Brooklyn. You will work with an interdisciplinary team to assess client needs, coordinate care, and promote retention in HIV care and self-management for Black and Latina women living with HIV. The role requires demonstrating cultural competency, coordinating with external services, and guiding clients through care plans while embracing a team-based approach within a unionized #J-18808-Ljbffr Housing Works, Inc.
