Opportunity: Physical Therapist Job Details: Evaluate assigned patients and develop, implement, and modify treatment plans in accordance with patient needs and physician direction Delegate and direct all discipline-specific therapy services for assigned patients Design functional treatment programs to support effective return to community activities Serve as an educator for patients, families, students, and coworkers Collaborate with interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, and planning for continuing services Complete comprehensive patient evaluations utilizing clinical reasoning to develop appropriate treatment plans and goals focused on progressing patients toward discharge Manage individual, concurrent, and group treatment sessions based on patient care needs Complete medical record documentation in a clear, concise, accurate, and timely manner including daily documentation, goal writing, functional quality indicator assessments, letters of medical necessity, and discharge documentation Ensure care is provided under signed, valid physician orders and within insurance approval Participate in student program as assigned, providing a positive learning environment with appropriate supervision and completion of student evaluations Maintain professional licensure in good standing and complete all state-mandated continuing education requirements Inpatient rehabilitation experience highly desired Minimum of 2 years of experience required BLS Certification required prior to first shift Must hold a valid, current state license Facility: This rehabilitation hospital is dedicated to providing high-quality inpatient rehabilitation services to patients recovering from critical illness and complex medical conditions.
